Welcome to Jewish Family Service! We sincerely appreciate your interest in our agency and wish to serve you in the most effective way possible.
When you are ready to set up your first appointment, please call our office at 402-330-2024. You will receive your Intake Paperwork online and will be able to fill out and submit them to us from the comfort of your own home, using a smartphone, tablet or computer.
If you choose to fill out paper forms in our office, please arrive at least 15 minutes before your scheduled appointment time.
The information requested of you in the Intake paperwork is thorough. Please know that we are asking necessary information required by your insurance company, to assist us with understanding your current symptoms and resulting diagnosis and for grant writing purposes.
Below you will find the documents, terms and conditions that you are agreeing to when signing your intake forms, including our cancellation policy. Please read those documents thoroughly before signing your consent form.

NEW CLIENT FORMS:
Call JFS at 402-330-2024 to request an appointment. Upon confirmation, you will receive the intake paperwork via email to fill out. If you do not have access to a smartphone or computer, you can fill them out in person before your appointment.
